Comprender por qu\u00e9 se produce es fundamental para poder prevenirla. Un estudio publicado en la revista <em>PLOS ONE<\/em> por investigadores del Centro M\u00e9dico de la Universidad de Duke en Durham, Carolina del Norte, ha arrojado nueva luz sobre uno de los posibles mecanismos implicados.<\/p>\n<h2>Bacterias y debilitamiento de las membranas: \u00bfqu\u00e9 encontr\u00f3 el estudio?<\/h2>\n<p>El equipo de Duke analiz\u00f3 muestras de membranas cori\u00f3nicas de 48 mujeres, incluyendo casos de PPROM prematuro y a t\u00e9rmino, obtenidas despu\u00e9s del parto. El objetivo era determinar si exist\u00eda una asociaci\u00f3n entre la presencia de bacterias en las membranas y el adelgazamiento del corion, la capa m\u00e1s externa del saco amni\u00f3tico.<\/p>\n<p>Los resultados mostraron que las bacterias estaban presentes en todas las membranas fetales analizadas, pero en mayor cantidad en las zonas de rotura y en los casos de PPROM. Adem\u00e1s, el corion era significativamente m\u00e1s delgado en el punto de rotura que en las zonas m\u00e1s alejadas, y este adelgazamiento fue mayor en las pacientes con PPROM que en las de t\u00e9rmino. Investigaciones previas del mismo grupo hab\u00edan documentado que el corion sufre mayor muerte celular cuando hay presencia de infecci\u00f3n.<\/p>\n<h2>\u00bfSon las bacterias la causa o la consecuencia?<\/h2>\n<p>Esta es la pregunta que el estudio no puede responder todav\u00eda. La correlaci\u00f3n entre mayor recuento bacteriano y mayor adelgazamiento del corion es clara, pero la direcci\u00f3n de la causalidad no est\u00e1 establecida. Las bacterias podr\u00edan debilitar activamente las membranas, o el debilitamiento podr\u00eda facilitar la colonizaci\u00f3n bacteriana. Probablemente ambos mecanismos interact\u00faan.<\/p>\n<p>Lo que s\u00ed est\u00e1 claro es que existe una relaci\u00f3n que merece investigaci\u00f3n adicional. El siguiente paso es identificar qu\u00e9 especies bacterianas espec\u00edficas se encuentran con mayor frecuencia en los casos de PPROM. Si se logra establecer un perfil microbiol\u00f3gico de riesgo, podr\u00eda ser posible dise\u00f1ar tratamientos preventivos con antibi\u00f3ticos o probi\u00f3ticos para las gestantes con mayor vulnerabilidad.<\/p>\n<h2>Factores de riesgo conocidos de rotura prematura de membranas<\/h2>\n<p>Mientras la investigaci\u00f3n avanza, los factores de riesgo conocidos que se asocian con la PPROM incluyen:<\/p>\n<ul>\n<li>Infecciones del tracto genital inferior no tratadas, especialmente vaginosis bacteriana.<\/li>\n<li>Tabaquismo durante el embarazo, que deteriora la estructura del tejido conectivo de las membranas.<\/li>\n<li>Gestaciones m\u00faltiples, que generan mayor presi\u00f3n mec\u00e1nica sobre las membranas.<\/li>\n<li>Antecedentes de parto prematuro en embarazos anteriores.<\/li>\n<li>Procedimientos invasivos como amniocentesis o cerclaje cervical.<\/li>\n<\/ul>\n<p>En IMFER prestamos especial atenci\u00f3n al control de las infecciones del tracto genital en las pacientes que siguen tratamientos de reproducci\u00f3n asistida, precisamente para reducir el riesgo de complicaciones durante el embarazo.
